Summary

Communication is understood as a process of transmission and understanding of information is the interaction of men. There are two types of verbal communication and non-verbal. We emphasize the importance of verbal communication in nursing, but don't ignore the non-verbal communication, as for the nursing Professional is of extreme importance in every possible way to interact with your desktop, because communication is a basic tool for the continuity of safe assistance. Let's give greater relevance to the documented and registered communication, since the records are valuable sources of evidence for the institution and stakeholders. In the healthcare process the chart has been the main source of information, and its importance to analyze the evolution of customer assisted, serves for the Defense healthcare professional, portray the quality of nursing care. In this context objective to describe the importance of verbal communication evidenced in nursing records. The study was conducted from September to December 2012, this is a literature review. Were used in the study books, articles and Local database. The results presented are referenced to the source authors.

Key words: communication, Evidence of nursing, Nursing's responsibility.

Introduction

Communication is understood as a process of transmission and understanding of information. Opportunities for Messaging. Is the environment in which people get along and then trigger actions, whether, by facial expression, opinions, feelings and ideas, where behavioral changes are expected to the receiver of the information by the received message (TREVIZAM et al., 1998).

Corroborating with Ferreira et al (2009) the communication is a process of interaction of men, as is the communication that share ideas, with acceptance or not of the people involved in the processes of issuing (the one that sends the message) and reception (the one receiving the message). There are two types of communication, verbal and non-verbal, as Silva (2005) verbal communication is one in which people express themselves through written and spoken language, and the non-verbal is the interaction of people without using words.

In this regard, we emphasize the importance of verbal communication in nursing, as Ferreira et al (2009) report is a basic instrument for the continuation of assistance to safe, effective and of good quality. For Saints and Balasubramanian (2010) is through communication that we are fulfilling our duties and we guarantee the efficiency of a work organized, for both, this information must be given in a clear, since the recipients of the message can present difficulties of understanding causing the communication is ineffective.

However not all ignore the non-verbal communication, as for the nursing Professional is of extreme importance to interact in every possible way with your work environment, whether they are: patient/professional-professional/professional, considering This type of communication can supplement the verbal communication (SILVA; 2005).

On the above, we will give greater relevance to the documented and registered communication, since the records are valuable sources of evidence for the institution and stakeholders, according to Possari (2005) in the healthcare process the file has been the main source of information, and its importance to analyze the evolution of customer assisted, serves for the Defense healthcare professional, portray the quality of nursing care, the degree to which the information is passed on and understood, reflect the degree of preparation of the professionals and promotes the continuity of work in different shifts and areas of assistance in the health institution, constitutes a basic and essential medium of communication between the entire multidisciplinary team, promoting a full and qualified assistance.

With the affirmative of the authors Potter and Perry (2004) the documentation reflects the quality of care and provides evidence of the responsibility of every team member. As a member of the team, the nurses need to communicate information about customers, accurately, effectively and on time. The result of the inadequate communication can affect the continuous treatment of the client. The chart is a valuable source of data used by all members of the health team, their purposes include communication, billing, financial education, evaluation, research, auditing and legal documentation.

In this context, it is relevant to the search for knowledge to enhance the practice of nursing with quality and evidence of services rendered. Objective to describe the importance of verbal communication evidenced in nursing records, which is an important document facilitator in the transmission of messages between health professionals.

Methodology

It is literature review study, conducted in the period from September to December 2012. Books were used. Articles, published and indexed in the databases considered by international centers of virtual Latin American literature and Caribbean Health Sciences (LILACS), Scientific Eletronic Librany Online (SCIELO), official reports from the library Virtual Health (VHL). Local database.

The following descriptors were used: communication; Evidence of nursing; Nursing responsibility.  Where some criteria were used for selected articles, as Portuguese-language publications and full texts, published in the period from 1998 to 2011, with specific topics and affinities of subjects with specific and relevant ideas from the authors.

Were included in this study three books, six articles, some articles of the code of ethics of nursing of the federal Council of nursing and some articles of the resolution of the regional Council of nursing in the State of Minas Gerais.

Results

Discussion

The communication process depicts the quality, creativity and professional skill in nursing, and have the ability to listen and to allow all persons involved in the transmission and reception of the message, explore with the capability information received. The nurse should take into account the verbal communication and to complement this, the non-verbal communication; targeting the message to be received properly and as desired (Saints; BALASUBRAMANIAN, 2010).

Ferreira et al (2009) underscores the importance of listening and speaking, as primary factors in communication and in the process of care. Declares that the nursing records when well described and effective communication, demonstrates the quality of care professional class differentiating. Therefore, the records is the means of interaction among professionals, ensuring the safety and quality of care.

According to Oliveira et al. (2005) we should consider that communication occurs in the process of transmission of the message to not lose the sense and be undermined, since, in the health context/communication assistance has to be planned and appropriately according to the degree each individual instruction. Corroborating Oriya et al (2004) says that communication is a fundamental part in the planning process for the achievement of the goals. Reaffirming, Oliveira et al. (2005) says that the communication reflects the degree of technical ability and quality of assistance in health services.

Silva (2005) points out that for an effective written communication, records should be objective and complete, should not be provided with the personal impressions. Describes that we don't have the facility to write or record information transcendental care (physical and psychic aspects, social and spiritual). Reports also that we do not have to report with objectivity the whole interaction built on process of care, and who also are not arranged in a language without variation and regular, hindering our objectivity in writing.

The handle and communicate are essential to do so in nursing, being relevant with regard to the care and humanized. Therefore, the systematization of care and the effectiveness of communication should be recorded to ensure that the information, once records are legal support to exercise the profession in nursing, contributing to the follow-up of care (Oriya et al; 2004).

Writing is a more elaborate thinking, where we filter the emotions, but that doesn't mean that in writing no emotions, these being transmitted by the scores. For the health team the chart is the most valuable document to exchange information, enabling the care, the healthcare value and the qualification of the services provided. Is still a document with purpose to support legally (Silva; 2005).

In the description of the authors Potter and Perry (2004) a record featured demonstrate the competence of the health team and how much she cares to offer a quality assistance and commitment with the institution raising the advantages for everyone: customer/ professional/institution. Where, to use the writing in a clear, concise and chronological. Log all information obtained or acquired after the care, routine care or specific. Leave no hidden information that might hinder the multiprofessional team service, this reflects our care and assists in the treatment, as well as reflect the bio-psycho-social conditions-spiritual. Use terms and acronyms of literature or standardized by the institution. At the end of each record not to forget the date, time, signature readable who ran the care and stamp.

In the view of Fernandez et al (2009) the written communication should be recorded, as it reflects the ability of the nursing. Security with that are passed on the information. The security and continuity of care. The interaction and integration and complementarity of information to another of different professionals. Writing serves to take questions and is the legal support that ensures the Professional do. Also involves the right of the client to know the entire care process that involves.

So it is necessary to describe some responsibility, duties and prohibitions of certain nursing COFEN resolution No. 311/2007, code of ethics of nursing professionals sorted in some articles:

Art. 25-Register in patient records the information inherent and essential to the process of care.

Art. 41-provide information, written and verbal, reliable and necessary to ensure the continuity of care.

Art. 68-Register on the chart and in other documents of information relating to the process of Nursing care of person.

Art. 71-Encourage and create conditions to register the information inherent and essential to the process of care.

Art. 72-Register the information inherent and essential to the process of care in a clear, objective and complete. Nursing is prohibited:

Art. 35-Register partial information and bungled on the assistance provided.

Art. 42-Sign the nursing actions that did not perform as well as allow their actions to be signed by another professional.

Still in deliberation COREN-MG 135/00, Norma in Minas Gerais State general principles for actions that constitute the nursing documentation:

Art 1-the record should be clear, objective, accurate, legible, and with no erasures.

Art 2-after the registration shall include the identification of the author

included name, COREN-MG and stamp.

Article 3-the record should appear in printed properly identified with data from the client or patient, and complemented with date and time.

Article 4-the record should contain subsidies to enable planning continuity of nursing care in different phases and for planning of multidisciplinary team care.

Article 5-the record should enable and encourage administrative and clinical elements to audit in nursing.

Article 6-the record should be part of the record of the client or patient and serve as a source of data for administrative, legal, research and teaching.

Article 7-The records can be of type: Manual; Written in ink and pencil never; Electronic; According to the current legislation.

Recommendations: hospitals have the authority to define their requirements, supporting documentation of nursing actions, so long as they are committed to ethical and legal standards and included in the manual of nursing service organization.

With the affirmation of Oriya et al (2004) the whole process of care must be evidenced, as this evidence provides effective communication between teams and contributes to the quality of the services provided. So '' the written verbal communication is also required, as is an important tool to support the process of care '' (Saints; Balasubramanian, 2010, p. 363).

Therefore communication is an indispensable factor for the development of health services, once ineffective, interferes in the process of your multidisciplinary care and continuity, worth mentioning as some references stressed the importance of permanent and ongoing education in routine to a hospital, since communication and evidence are educational processes that qualify the services provided. Negreiros et al (2010) States how important it is to invest in continuing education, with the theme: communication, to show the effectiveness of a message when well conveyed, understood and highlighted.

Corroborating Oriya et al (2004) says that continuing education referred to the communication is of paramount importance to the visibility of the nursing as specific needs and inherent, clarifying the questions according to the degree of understanding of the information transmitted and received. So '' communication is interference factor in the dynamics of operation of any health institution, being instrumental in the development of work '' (Saints; Balasubramanian. 201, p. 365).

Final considerations

It was noticed that after the literature review, some authors have highlighted negatively the process of communication between a multidisciplinary team and the difficulty of understanding the information described. These same authors declare the importance of continuing education as a process of qualification of the services provided.

Thus, after a discussion, it is necessary to acquire knowledge and update itself so that the process of communication is not lost within the institution, since the written communication is the most propitious to take the questions of who receives the message, as in the communication process there are flaws with regard to suitability of formal language, grammatical accuracy, brevity, readability, identification and technical terminology.

Therefore, the documentation and reporting of quality are needed to ensure effective and individualized care. A complete record, real, updated, accurate and organized, is a facilitator of teamwork, being of interest to all professionals that the expected results are achieved with success, when it comes to customer needs.

So, it is worth mentioning that this study has allowed me to better understand the triggering of the communication process and the importance of those involved in this process the transmitter and receiver of the message and how essential the communication is based on evidence.
